How to Enter

Entry forms are made available from the coaches 3 weeks before each of the splash nights or you can download an entry form off the Jasi website.
Complete the form putting in the following information;

Name:

Please put both your surname and your first name.

Date of Birth: Day / Month / Year

Your age group will correspond to how old you will be on the day of the competition.

Events:

Tick the box that will correspond to the events you wish to enter.

Entry Fee:

Tally up the number of events you are going to swim in and multiply this by the entry fee of $1.50 per event. 4 events = $6.00.

Posting Your Entry:

Put the entry fee and form in an envelope and post it in the Splash Night Entries box on poolside, or email to peter.burgon@ccc.govt.nz by the Wednesday prior to the Splash Night.
